Kamehameha Schools Physical Evaluation Form
All athletes must submit an updated physical (not older than 13 months) annually to their respective health rooms.

Infinite Campus
Students interested in athletics must complete the Athletic Agreement and Release form on Infinite Campus. All required signatures must be obtained in order for required forms to be marked as "Completed."
